About SAP Evaluations
A Substance Abuse Professional (SAP) is a DOT-qualified person who evaluates employees who have violated DOT drug and alcohol regulations. The SAP determines what assistance the employee needs and provides follow-up evaluation.
You need a SAP evaluation if you’ve violated DOT drug/alcohol testing regulations, including: a positive drug test, a positive alcohol test (0.04 BAC or higher), refusal to test, or other violations of DOT testing rules.
Yes. Your evaluation and all related records are strictly confidential and protected under federal regulations. Information is only shared as required by DOT regulations.
The Process
The initial evaluation typically takes 1–2 hours. The total return-to-duty timeline depends on the recommended treatment/education, which varies by individual. Many clients complete the process within 4–8 weeks.
The SAP conducts a comprehensive face-to-face clinical assessment. You’ll discuss the circumstances of the violation, your history, and any substance use concerns. The SAP then makes recommendations for treatment or education.
Not necessarily. The SAP’s recommendations are based on your individual assessment. Some individuals may need outpatient treatment, others may need education courses. The SAP tailors the recommendation to your situation.

DOT regulations require face-to-face evaluations. However, we offer flexible scheduling to accommodate your needs.

Return to Duty
Many employers support employees through the return-to-duty process. The SAP process is designed to help you return to safety-sensitive duties, not to punish you.
Once you complete the SAP’s recommendations and pass a return-to-duty test, you can be cleared to return. The timeline depends on your individual treatment plan.
DOT regulations require a minimum of 6 directly observed follow-up tests in the first 12 months after returning to duty. The SAP may require additional testing for up to 60 months.
